The following file is for NEW or EXPIRED Notaries. Please note that you must have completed and passed the written examination prior to sending in your paperwork.

The follow package is for RENEWAL applicants. The Secretary of State requires that any renewal applicants be submitted to their office no later than the date of expiration. If you have already expired please use the new Notary application form.
